Circulating tumour cells (CTCs) as a novel analyte for monitoring HER2 status

Overcoming limitations of tissue-based tests

Testing of human epidermal growth factor receptor 2 (HER2) is recommended for all newly diagnosed breast cancer cases and at disease recurrence 1-3. Our new HER2 assay identifies and quantifies HER2 protein expression and gene amplification in CTCs from a single blood sample. This approach may overcome the challenge of local or distant recurrence where tissue biopsies may be out-of-date or not accessible in inoperable patients.

Association with poor prognosis

The presence of HER2-positive CTCs in patients with breast cancer, particularly those with initially HER2-negative primary tumours, has been associated with poor prognosis 4-6.

Critical role in disease progression

Studies have shown that patients with HER2-negative tumours, but HER2-positive CTCs, have shorter survival, suggesting that HER2 expression on CTCs plays a critical role in disease progression 4,7.

Predictive value for treatment response

Patients with HER2-negative primary tumours, but HER2-positive CTCs, who received lapatinib in the DETECT III trial, showed improved overall survival compared with those who did not receive the targeted therapy 6.

Our HER2 assay can help identify individuals with HER2-positive CTCs and prior HER2-negative primary tumours, and may assist in identifying those who could benefit from HER2-targeted treatments within a clinical trial.

Potential prognostic marker for longitudinal monitoring

HER2 status changes with breast cancer progression, with enrichment of HER2-low tumours in advanced stages 8. HER2-positive CTCs have been detected across early and metastatic stages of breast cancer 5, suggesting their potential as a prognostic marker throughout the disease course 9.

Our new HER2 assay has the potential to enable more accurate patient stratification for clinical trials and subsequent longitudinal monitoring.

Novel HER2 assay for CTCs

Our new assay identifies and quantifies HER2 in epithelial, mesenchymal and epithelial-to-mesenchymal (EMT) CTCs from a single blood sample. HER2 protein expression is identified via immunofluorescence (IF) staining, and HER2/neu gene amplification is quantified via fluorescence in situ hybridisation (FISH). Available through our GCLP-compliant laboratory, for research use only.
